Application requirements:
(a) Completed application form. (b) Valid passport. (c) Proof of sufficient funds to cover stay. (d) Two passport-size photos. (e) Itinerary of trip. (f) Return ticket and documents to enter any other country to which travel is planned. (g) Stamped self-addressed envelope if applying by post. (h) Proof of employment or enrolment in University. (i) Proof of hotel booking or letter of invitation if staying at a private home.

Single-entry: $30; Multiple-entry: $40 (3 to 6 months); $60 (6 to 12 months)

The Visa is valid for Usually 3 months. Applications for extension should be made to the Director of Immigration.

but i called the us embassy they gave me a number 2 call i because i need 2 get her passport but i wanted to know if i have to take 2 flight out to get a next birth certificate.

No. Contact the hospital and they can give you teh requirements needed to mail you a copy of the birth certificate. Had it done in PA when mine was misplaced and it was simple and cheap as long as you can prove you are the parent or person who lost it
